Catholic Charities receives $10K from Dignity Health

Catholic Charities Community Services received a $10,000 Community Grant from Dignity Health. The funds will be used to provide counseling to patients with chronic medical conditions who are in need and who receive treatment from Dignity Health’s internal medicine clinics.

Dignity Health’s – St. Joseph’s Hospital and Medical Center Community of Care grants through collective impact funding seek to increase access to quality care for people in low-income and under served communities improve their lives. Focus is on improving the health conditions for those individuals who are in need and would benefit from an integrated approach by improving chronic conditions.
